Preliminary Screening Assessment

Welcome to MindScope's screening questionnaire. This set of 20 questions is designed to gather information that may help to determine if an Autism and/or ADHD assessment might be beneficial for your child.

Please note that this is a preliminary screener only, and is not intended to provide diagnostic information.

Does your child interact differently from their peers, such as struggling to initiate or maintain conversations, finding group settings overwhelming, or preferring structured interactions (e.g., shared interests, role-playing, one-on-one conversations)?

Does your child carefully watch and copy others in social situations to figure out what to do, but struggle to adjust when social expectations change or become unpredictable?

Does your child struggle to recognise unspoken social rules, such as how close to stand to someone, when a conversation has ended, or how to tell if someone is joking or serious?

Does your child become exhausted or need alone time after social interactions, even if they seem confident or engaged at the time?

Does your child communicate in a way that is noticeably different from their peers, such as speaking in a formal or scripted way, using advanced or precise vocabulary, repeating phrases, or having a unique rhythm or tone of voice?

Does your child become intensely focused on specific interests, talking about them often or engaging in them for long periods?

Does your child struggle with unexpected changes in routine, becoming distressed or needing extra time to adjust when plans change?

Does your child have strong reactions to sensory experiences, such as covering their ears at loud noises, avoiding certain clothing textures, or being extremely sensitive to lights or smells?

Does your child experience meltdowns or shutdowns when overwhelmed (e.g., crying, yelling, running away, or becoming silent and unresponsive when distressed)?

Does your child find mainstream school challenging due to difficulties with focus, social demands, anxiety, changes to routines, or feeling exhausted by the school environment?

Does your child frequently struggle to stay focused on tasks or activities, even those they enjoy (e.g., listening to stories, completing homework, following multi-step instructions)?

Does your child often seem to "zone out" or not hear when spoken to directly, even when there are no obvious distractions?

Does your child frequently start tasks but leave them unfinished, even when reminded (e.g., forgetting to complete schoolwork, chores, or daily routines)?

Does your child frequently misplace or lose things needed for daily activities (e.g., school supplies, toys, clothes, or important belongings)?

Does your child often struggle to stay organised, such as forgetting important dates, rushing at the last minute, or needing frequent reminders?

Does your child have difficulty sitting still or staying in one place for long periods (e.g., leaving their seat at school, fidgeting, needing to move during meals or homework)?

Does your child frequently interrupt conversations, talk excessively, or struggle to wait their turn when speaking?

Does your child often act impulsively without considering consequences (e.g., running into the street, climbing on unsafe structures, blurting out answers, or making risky choices)?

Does your child struggle with waiting their turn in games, group activities, or everyday situations (e.g., waiting in line, taking turns during play)?

Does your child become easily frustrated or emotionally overwhelmed, especially in response to unexpected changes, difficulties, or when things don’t go as planned?
